Nurse Navigator

My role as a Patient Navigator is to ensure that you/your loved one is receiving the best possible health care within a timely manner; all while keeping you and your loved one informed and involved throughout the process. Living far away from an aging or ill parent can make you feel helpless. As a Patient Navigator, it is my job to ensure your loved one is, not only receiving the best care, but that you remain in the loop. Allowing a loved one to remain in their home is always ideal, but not always easy. Part of my job is to ensure your loved one can safely age in their safe, comfortable space.
